PST (GMT-8), I would describe my activity as fairly active. I am a college student, so that will come first but I also plan to make a commitment towards EMS and being an active member who will help the faction grow. I’d say I can commit at least 2-4 hours a day to YRP.

My motivation to apply to Norowarejima’s Hospital Faction is to be an active and committed member of the team. I’d like to help and watch the server grow and progress. As someone who previously roleplayed as a surgeon on a few different servers, I miss medical roleplay severely.

Do you have any prior experience with MedicalRP?

I do have prior experience with MedicalRP. I was a surgeon for about 8 months on SRP and the clinical lead for the surgeon faction for 4 months. I was the top surgeon for several months, reaching quota every month without fail. Along with playing as a surgeon on a separate server for 6 months. Not only have I put together a portion of the previous medical roleplay documents, but I have the knowledge and means to put together another document to set up the hospital. I’ve also worked in my day to day life as a pharmacy assistant, knowing the common medications needed for medical roleplay. 

 

Are you aware that with MedicalRP comes the possibility of encountering possibly triggering topics?

Yes, I am aware that possibly triggering topics are potentially a part of MedicalRP and am willing to encounter them.

How well can you coordinate with a team during Roleplay?

I can coordinate fairly well with a team, as I have experience with working in several teams in roleplay. I’m confident in my skills to lead and provide an engaging and welcoming community.

MEDICAL TRIVIA

 

(PHYSIOLOGY)

If someone is experiencing a reaction to food poisoning, what is the correct way to manage and treat the symptoms?

  • Hook the patient to a heart monitor, then use a lubricated tube to insert it into EITHER their mouth or nose. Choose one. Not both.
  • Move the tube further down the esophagus to their stomach. Aspirate some of the contents (suction out contents in the area) to ensure you’re in the right spot!
  • Use a funnel tube or insert 200mL of Saline down the tube alone (This drains the fluids as you suction out contents into a buckshot pan! Best to keep that on the floor by the bed.)
  • Repeat flushing the saline through the tub until the contents and liquids from the patient until it runs clear in the pan.
  • RECOVERY!
  • Food poisoning = Electrolytes, minerals, and vitamins




What procedure would you follow if you witness a patient experiencing a seizure?

  • Keep the area clear
  • Clear hard/sharp objects away from them
  • Ensure their airways are clear, cut or remove any objects that may potentially restrict breathing.
  • Do NOT restrict their movements, simply move them carefully down and onto their side.
  • Time the seizure.
  • Once the patient stops seizing, administer clonazepam.
  • Run a CT scan.



What procedure would you follow if a patient that has entered the hospital has a broken forearm?

  • Push pain relief through the I.V.
  • Move the patient into the X-Ray room.
  • Run X-Rays on the patient (See: X-ray protocols)
  • Have the patient remove any and all metals (Piercings, belts, etc.)
  • Have the patient change into a hospital gown.
  • Place the protective vest on yourself and the patient.
  • Lay the patient down on the X-ray table.
  • Maneuver the X-ray machine to the site that needs X-rayed. 
  • Turn off the lights in the X-ray room.
  • Move to the radiology room and take images of the site. Have the patient /it any breaks, fractures, foreign objects, etc.
  • If cleanly broken:
  • Apply general anesthesia to the area of the break.
  • Set the bone back in place.
  • Wrap a layer of soft fabric (a stockinette) around the broken bone.
  • Roll cotton over the stockinette.
  • Soak fiberglass strips in water then lay them over and around the broken bone, layering it until the entire break is covered.
  • Apply the bandage! Ask what color your patient would like from our selection.
  • If fractured, surgical procedures may be needed. 
  • Hook the patient up to a heart monitor and an O- blood bag. 
  • Set the anesthesia mask over the patient’s mouth and nose, administer anesthesia, and wait for the patient to fall asleep.
  • Cover the break site in iodine using a cotton swab.
  • Make an incision using a scalpel above the break site.
  • Depending on severity, use plates or rods to set the bones back in place.
  • Suture the incision site back up, and snip off excess Vicryl.
  • Apply anti-biotic and anti-scarring cream.
  • Wrap a layer of soft fabric (a stockinette) around the broken bone.
  • Roll cotton over the stockinette.
  • Soak fiberglass strips in water then lay them over and around the broken bone, layering it until the entire break is covered.
  • Remove the anesthetic mask from the patient.
  • Wait for the patient to wake up from anesthesia.
  • Apply the bandage! Ask what color your patient would like from our selection after they’ve woken up!



What procedure would you follow if a patient enters the hospital struggling to breathe?

First, administer Albuterol through an inhaler to relieve symptoms and test for asthma using a spirometer. Then, assess and rule out any causes of trouble breathing, including heart attack.

  • Clear their airways, removing any tight or restrictive clothing or accessories. 
  • Run an EKG (Electrocardiogram), setting the sticky patches (electrodes) on the chest, arms, and legs, and wait for the EKG to show up on the monitor or printed paper.
  • Run a blood test to check for proteins that may indicate heart damage or heart attack (cardiac markers). Have the patient /it whether or not there are present markers
  • Run a chest x-ray
  • IF the EKG and blood test prove a heart attack, a coronary catheterization (angiogram) may be needed: Insert a long, thin catheter into an artery, usually in the leg or arm
  • Allow the dye to flow through the catheter to help arteries and blockages show up more clearly on a CT or MRI scan
  • Once the patient is stablized, prescribe Aspirin and angiotensin-converting enzyme (ACE) inhibitors like lisinopril (blood pressure medication) to tame the symptoms.
  • IF a blocked artery occurs, surgical procedures may be needed. Contact a surgeon to set up the procedure. 

(PSYCHIATRY)

A patient is experiencing a panic attack before an important procedure. How would you assist the patient?

To assist the patient, I would offer them medication to alleviate the symptoms of anxiety, such as an SSRI like Escitalopram for example. I would also sit them down, and offer an ice pack or cool water to sip on, while calmly addressing the need for the procedure. I would also explain the procedure step by step, this ensures the patient that I know what I'm doing and have the knowledge and expertise to perform the procedure. 

 

 A belligerent patient in the middle of a psychotic episode stumbles into the emergency room. What should you do?

If a patient proves as a threat to themselves or to others, restrainment may be needed. However, the best course of action is to try and coax them into a relaxed state, by either talking them down or offering medication to them. If the patient is still belligerent, forceful treatment of medication may be required. 

 

Which neurotransmitter is most commonly associated with depression?

Serotonin is the neurotransmitter most commonly associated with depression, which leads to the need to take Selective serotonin reuptake inhibitors (SSRIs).

PTSD stands for post-traumatic stress disorder. Commonly mistaken as just a symptom of a soldier, it applies to all variations of trauma. Whether it be from emotional, physical, or psychological trauma. PTSD is the result of trauma, often leading to vivid flashbacks or states of fight, flight, or freeze. Disassociation is often a symptom of PTSD, which breaks off into several different branches. It can take months to years for symptoms to appear after a traumatic event.

I am a Senior surgeon from Osaka. I am writing to you to apply for the position of Norowarejima’s Hospital Director. I recognize the strength and resilience required for the task at hand and am simply asking for a chance to interview me for the position. Having previously worked as a surgeon based in Osaka, I understand the pressure and expertise needed for a large hospital such as Norowarejima’s. I have my PhD in phisiology, along with my master’s in psychology and chemistry. I’ve been an academic for as long as I’ve been alive, aiming for research and aiding those in need.

 

I had previous work experience as a pharmacist technician before I left for my studies. After serving my residency in the Osaka City Hospital, I worked as a surgeon for twenty-two years. I’d like to bring my skills to Norowarejima’s Hospital, aiding in the organization and preparation of the city’s faculty. I’ve worked as a clinical lead of my surgical teams for many years, understanding the need for teamwork and collaboration.

 

I have also prepared a series of procedures and protocols for the hospital to use and improve upon. As a starting block, a foundation, rather than the solid rule and basis I would like to run the hospital as. The Hospital Director takes a seat at their desk, clasping their hands together with a calm gaze as they greet the individual. Their desk is clean and pristine, aside from a half-filled tea cup that continues to steam, giving the room a lovely aroma.

 

A half-baited breath is exhaled, a small smile presenting on her lips. Her eyes seemed caught on the steam, before bringing her attention to the Director, maintaining steady eye contact with them. Seiren's back straightens out, folding her hands in her lap.

 

“Good afternoon. I will be conducting your interview today. Could I have your full name as well as your age and date of birth, please?”

 

There’s a curt nod offered their way, “Good afternoon, my name’s Seiren Tsuchiyo, I am fifty years old, and my date of birth is December 23rd, 1975.” She supplies. There's not much visually to prove that statement, besides the smile lines present on her face.

 

“Very nice. Now, could you tell me a little more about yourself? Do you have a family, perhaps some motivations behind why you’d like to work here?”

 

“I don’t have a family here, I do have..” A beat, “Close friends here I would like to stay near.” She lets out a brief, nervous laugh, brushing some hair out of her face. “However, my main motivation is to be a part of something bigger, I’d like to provide my skills to Norowarejima’s expansive growth.” 

 

“Would you like us to refer to you by your surname or first name? As well as what honorifics you’d like.”

(San, Chan, Kun, Shi, etc.)

 

“I prefer to be addressed by my surname, for professionalism’s sake. Tsuchiyo-San is just fine. Unless we’re close!” Seiren bears a grin, “Then I don’t mind being Seiren.”

 

The director nods in affirmation, now understanding the type of person the individual was and readjusts their posture, taking a sip of that aromatic green tea before continuing to speak.

“It is nice meeting you, then. Now, Let us move on to some more important questions regarding your role.”

 

Careful eyes examine the director, something is so reaffirming about their calm demeanor, it only fuels her confidence. She nods, ready to answer to the director’s whims.

 

“A patient arrives with a severe injury and is panicking uncontrollably. Walk me through how you would handle this situation.”

 

“At first, I would examine the wound. Staying calm is essential, it ensures the patient that there isn’t a need to panic. However, I would offer medication such as a sedative or anti-anxiety medication like Escitalopram. To treat the onset of panic, an ice pack to the back of their neck to trigger a physical response instead of a psychological or emotional response. I would then establish the box-breathing method, to help calm the patient before we move to assess the issue at hand.”

 

“During an emergency, if any other hospital staff are unavailable however there is a patient in need, can we trust you to stabilize them or treat them to an acceptable degree?”

 

“I’m not a rookie..” A small laugh is stifled, before speaking again, “I’ve had years of covering for staff and taking care of patients. It’s pressure, but it's not something I worry about... I'm confident in my abilities and my level of expertise to take care of patients in my coworker's steed.”

 

“Could you run me through on the plethora of equipment you may find within your personal office if you are to be given the role of a resident? Just simply medical equipment.”

 

“A stethoscope, mm.. thermometer, stadiometer, scale, blood pressure cuff, spirometer, otoscope, laryngoscope, and a rubber hammer, gauze, bandages, a bottle of saline all seem like the simple medical equipment I’d expect to have on hand as just a resident.”

The Director hums in curiosity after quickly scanning over the individual’s application, noting that they are wishing to take over the Hospital Director position

“A very noble role. How interesting. Are you aware that if you are to take over my role within the Norowarejima Emergency hospital, you will also be taking over my previous work and responsibilities?”

 

Tsuchiyo takes in a deep breath, exhaling slowly before opening her mouth to speak. “I’m fully aware of the daunting role I’m applying for, and the tasks I’m expected to achieve. I commend your previous work, it’s not a simple task, but I’m willing to carry it on.”

 

They clear their throat, collecting a document with a collection of questions they then begin to recite.

 Her fingers lightly tap on her knee, anticipating and preparing herself for the potential onslaught.

 

“Why should you receive this position above anyone else interested?”

“I am an asset. As egotistical as it possibly sounds, I didn’t get anywhere by simply waltzing through life. I worked hard and have gone through my trials and tribulations, I know what it takes to work and perform with others. I can see myself in a position of leadership.”

 

“Now. As the director you will be in charge of not only ensuring your patient’s safety, but managing the safety within the workplace. Let's see.. How would you address a conflict between two Hospital Staff members?”

 

“That’s a tricky one..” She admits, letting out a quiet hum, “Between two staff members, I would gather information from both sides. To diffuse the tension, I might delegate the staff members to different wings, or rather ensure that the issue is settled so it no longer interferes with work. It’s crucial to make sure our team is working in tandem, rather than against each other.”

 

“The hospital receives a lawsuit from a patient that believes their operation or treatment was not properly executed. Who would you reach out to and how would you handle the lawsuit accordingly?”

 

Her brows furrow, despite this, “I would reach out to a lawyer, along with the higher-up in charge of the staff member who worked with the patient. As for the lawsuit, I believe it’d be best to try and settle the lawsuit before it came to court, by offering monetary compensation or punishing the staff member involved in malpractice. If it comes to court, it’s best to be apologetic and understanding towards the patient. They were entrusted in our care afterall, and are our responsibility.”

 

“The Inarigetsu Academy brings in a collection of students studying in the medical field and they wish to witness an operation for an assignment. How will you respond and thus, how would you ensure everyone’s safety and focus during the operation?”

 

“I would accept, although, it would require consent from the patient, along with consent from the dean of the Inarigetsu academy. A waiver of sorts, ensures that there will be no lawsuits or students holding the hospital liable for psychological damage. Operations are not for the weak-stomached. I would also make sure there’s a wing for observation in the surgical rooms, for students to observe without interfering in the process. While another surgeon can explain the procedures and protocols going on.” 

 

“What unique Contributions can you bring to our hospital in my stead?”

Seiren seemed... Ecstatic, almost beaming before she presented her own stack of documents. “It’s not finished, nowhere near finished, I still have the psychiatric procedures to complete, along with a few other procedures and protocols to finish. But-” She gestures for the director to look through. “I prepared this for this position, building blocks for how I’d like to oversee the hospital and the procedures done under my care.”
