My timezone is AEST/AEDT (depending on what time of the year) otherwise known as GMT+10/11

 

As for my activity, I have hit a period where I will be commencing most of my studies online so therefore a lot of free time has opened up for me for the next 6-8 months. I can allow that time to be well spent in the faction and on the server as a whole. So to put a scale to this, I can guarantee my activity will be at least an 8/10 majority of the time. If anything unexpected pops up or I have more work hours than I usually do I will obviously follow the right paths to voicing that such as an inactivity log.

 

What is your motivation to apply to the Norowarejima Hospital Faction

 Most that know me on a personal level are well aware how in depth I am when it comes to all things medical related - To put it short I am a medical student studying a course in radiology and medical screening. Alongside that I also do tutoring for subjects alike and specifically specialise in the human science areas. So to say this faction is ‘up my alley’ would be a total understatement. 

 

If I stretch away from my passions for this topic, my second big motivator is that this is an amazing project in the works that I would absolutely love to contribute to and be a part of. The entire concept of this server and more importantly the lore and ideas for this faction have me well intrigued and I feel as if I have a lot I could bring to the table and return to the community in the behind the scenes and in character development of EMS in the future.

Do you have any prior experience with MedicalRP

I have plenty of experience with MedicalRP, being that I was a part of EMS on another RP server known as SRP for a little over a year - I feel it’s safe to say I have encountered a lot of different scenarios with my time spent in that faction. Within that year I moved through all different departments of EMS such as starting out as a surgeon, doctor and then finishing as a psychiatrist, so I had a vast amount of different training sessions and I still do have a collection of notes on hand from that. Not only that but I did also help write out the textbooks for those training sessions alongside other individuals and worked my way up through the roles just shy of what would be considered as a ‘Higher-up’, that also meant I was training all the new residents that got accepted!

 

Another note to add is that being within each section had allowed me to experience different amounts of lengths and details to go into when it came to the initial rp scenarios and understood how the faction was run as a whole.

Yes I am well aware that there is the possibility of encountering triggering topics but I know I am able to handle it respectfully and maturely and that I am well equipped to move on without problems. 

 

How well can you coordinate with a team during Roleplay

I believe I am naturally a leader when it comes to working and coordinating in teams, however that is not without the balance of teamwork. As cheesy as it is, I strongly stick behind the whole ‘there is no I in teamwork’. During roleplay, I feel as if I have all the right skills and experience already to coordinate well and efficiently and create a welcoming and fun community both in and out of character.



MEDICAL TRIVIA

 

(PHYSIOLOGY)

If someone is experiencing a reaction to food poisoning, what is the correct way to manage and treat the symptoms

The correct way to manage and treat an individual with food poisoning is to begin by laying the person down for rest and monitor their current conditions. It is important that they remain hydrated using intravenous fluids and/or electrolytes to avoid any kind of dehydration. 

 

If the individual has eaten certain toxins that are worsening their symptoms, usually the next step is to flush the stomach using a process called a gastric lavage. This is done by using a tube through either the mouth or nose to then flush out the contents with a saline solution. Then activated charcoal can be administered to absorb the remaining toxins.

 

What procedure would you follow if you witness a patient experiencing a seizure

At first you would move any objects that could endanger them and provide support by placing a pillow or something soft underneath their head as well as loosening and tight clothing they have. The next steps are to ensure the airways are all clear and that the patient has clear IV access.

 

Once the above is all clear the patient should be administered of benzodiazepines and continued to be monitored until they come back. If IV access is unattainable then other medications such as  lorazepam or midazolam can be used in place via an intramuscular injection.

 

Treatment to prevent further seizures will then be prescribed before leaving.

 

What procedure would you follow if a patient that has entered the hospital has a broken forearm

First and foremost the patient requires an X-Ray over the arm to identify what kind of fracture it is and decide whether it will require surgery to fix or simply just a cast.

 

Typically if the breaks clean and not displaced or is just a small hairline fracture, it only requires a cast and some pain medication. The casting process starts off by slipping a cotton slip over the limb with the fracture (in this case it is the arm) which you then follow by a thicker roll of cotton padding, 1 or 2 layers of this will create the perfect cushioning before finally layering on a roll of fibreglass bandage cast tape. The cast tape layer should consist of 3-4 layers to have a solid and structured cast with the ability to still have some form of mobility.

 

What procedure would you follow if a patient enters the hospital struggling to breathe

The first step is to quickly sit or lay the patient down and uncover what is causing issues to the airway, if the airway is blocked then unblock it and provide the patient with a breathing treatment such as albuterol nebulization. Then continue to monitor the patient's vitals until they are back to being stable.

 

In the worst case scenario where the patient’s symptoms worsens and they become unable to breath on their own then intubation is required right away. This procedure is where the patient is put to sleep whilst a tube is inserted down the trachea to help the patient breath. Intubation can also be required if something blocking the airway cannot be removed in a timely manner or needs extra force to remove as it can bypass the blocked portion of the airway.

 

(PSYCHIATRY)

A patient is experiencing a panic attack before an important procedure. How would you assist the patient?

It is important to remain calm yourself, there is no point of even attempting to reassure the person if you yourself aren’t okay. The first step is to help the patient focus, this could be asking them to repeat tiring or awkward tasks such as getting them to raise their hands over their head. Another way of regaining their focus is a rule called 3-3-3. Ask them to look around and identify 3 objects, 3 sounds and 3 body parts. The next step is slowing down their breathing which can be done by basic breathing exercises such as breathing with them or slowly counting to ten. Once their condition is more stable you reassure the person and explain that these symptoms will pass and that the procedure will do no harm to them. If the patient fails to compose themself then a small dose of diazepam is recommended to help relieve the symptoms of anxiety and get the patient into a more stable mindset before continuing on with their treatment.

 

A belligerent patient in the middle of a psychotic episode stumbles into the emergency room. What should you do?

Quite like the situation above, you would ensure to remain calm at all times and approach the matter in a safe and calming manner. If they pose a danger to the others around them then ensure that they are to slowly back away into another safe space whilst the patient gets the help they need.

 

Also similar to the scenario above, you’d try your best to talk them through things and get the patient in a more stable and calm condition. It is important to try and tailor the confrontation to how they are acting and what their mannerisms are like. If they are presenting with an aggressive episode it is recommended that you do not tackle this on your own and you have the aid of one or two doctors (preferably psychiatrists).

 

If the episode continues and worsens throughout the confrontation and prior strategies have not worked, as a last option you would inject them with a dose of midazolam and have them moved into an isolation room or padded room, until the episode tides over. Follow up sessions with a psychiatrist are generally required after a situation like so and medication would be prescribed.

 

Which neurotransmitter is most commonly associated with depression

Serotonin is the neurotransmitter commonly associated with depression. A significant drop in Serotonin levels is the result of depression, hence antidepressant medication usually contains Serotonin Reuptake Inhibitors.

 

What does the Acronym [PTSD] stand for, and what does it entail

PTSD stands for ‘Post Traumatic Stress Disorder’ and is a disorder when an individual has either experienced or witnessed a very difficult and traumatic event that causes nightmares, flashbacks, extreme anxiety and uncontrollable continuous thoughts about the particular event. Patients usually tend to avoid specific locations or triggers of the event which then results in issues in returning to everyday functioning.

THE INTERVIEW

The Hospital Director takes a seat at their desk, clasping their hands together with a calm gaze as they greet the individual. Their desk is clean and pristine, aside from a half-filled tea cup that continues to steam, giving the room a lovely aroma.

Aikawa followed swiftly behind the Director, taking a seat of her own. A friendly smile was offered as she reached down, gently placing a small folder on the table. Adjusting her posture in her seat, she’d finally set her hands in front her - awaiting the start of the interview.

 

“Good afternoon. I will be conducting your interview today. Could I have your full name as well as your age and date of birth, please?”

“Good afternoon to you too! My name is Xingfei Aikawa, I am 35 years old - my birthdate being the 14th of February, 1990” 

 

“Very nice. Now, could you tell me a little more about yourself? Do you have a family, perhaps some motivations behind why you’d like to work here?”

Aikawa simply nodded her head, her lips pursing right before she spoke. “I am a single mother to my two huskies if that counts as a family to you!” She’d offer a small laugh before moving on. “My family are all from Shanguo, China. I had moved to Japan just after I had completed high school to further my studies in a new environment.” There was a brief pause as her eyes slowly moved to a nearby window then back to the director. “To be completely honest my biggest motivation to work here at Norowarejima; besides the job opportunity, is the unique allure this entire island presents itself with. I find it quite intriguing and I would love to learn more.” Her fingers quietly tapped against the desk as she let out a small hum. “Of course it has also been my dream to work in a top position such as yourself, I really feel I could work my magic in this hospital you know? Meeting the residents and learning the true beauty of this entire island would simply be easier with the contacts I’d have as director” There she was, back on that same tangent as before. Rambler much. . was she simply a mad woman?! “Ah. . I do love to help people, that being the main part of this role and everything-” She cut herself off finally.

With a friendly smile, she’d clasp her hands together. “I prefer for my last name to be what others refer to me as- at least until we are on a more personal level. So Aikawa-San is fine and preferable for the time being!”

 

The director nods in affirmation, now understanding the type of person the individual was and readjusts their posture, taking a sip of that aromatic green tea before continuing to speak.

“It is nice meeting you, then. Now, Let us move on to some more important questions regarding your role.” 

Aikawa nodded her head, carefully pulling out a few documents from her folder in front of her, having them at the ready if required. “Mhm! Ask away” Her demeanour was neutral, although occasionally her eyes would fall down to their green tea, oh how she wished she bought one on her way here. . . “I’m ready whenever you are”

 

“A patient arrives with a severe injury and is panicking uncontrollably. Walk me through how you would handle this situation.”

All thoughts about the green tea had simply diminished as her posture propped up and her mind went straight to the question, she was still a scholar at heart. “First thing I do is get them into a quiet treatment room to fully assess their condition, as I am observing the kind of injury they have obtained I will calmly talk them through what I see. I’d then reassure them that they are in the perfect place and that we will in fact take care of them efficiently. If this is unsuccessful, I’d administer them with a small dose of diazepam - presumably after or alongside the appropriate pain medications” She’d pause briefly for a moment, letting her response slowly be absorbed before moving on. “The diazepam provides as an aid in relieving symptoms of anxiety, a weaker sedative to allow us to continue on with their treatment in a calming manner”

 

“During an emergency, if any other hospital staff are unavailable however there is a patient in need, can we trust you to stabilize them or treat them to an acceptable degree?”

She’d let out a short chuckle at that question, slowly nodding her head. “Well of course! I didn’t go through years of studying just to stand around and be useless in times of need” Her tone made sarcasm rather apparent for that statement. “But yes, as mentioned in my cover letter, I have years of experience, more so emergency medicine in particular, so I am able to provide the appropriate care to treat or stabilize them with exceptional outcomes.” She’d slide over a few documents regarding her qualifications, solidifying her honesty in that answer. “All my certifications are listed and signed here”

“Could you run me through on the plethora of equipment you may find within your personal office if you are to be given the role of a resident? Just simply medical equipment.”

Aikawa pursed her lips, fingers gently tapping on the table once more. “Ahh back in my resident days I remember having equipment such as thermometers, stethoscopes, blood-pressure cuffs, rubber hammer, syringes and of course a small kit which would include gauze pads, bandaids, swabs, alcohol wipes, cotton applicators and tongue blades. For communication means, we did have either work phones or handheld radios. If we needed anything else we usually made our way to the equipment room and with the permission from a senior we’d take what was required!”







[HOSPITAL COORDINATOR APPLICANT ONLY]

 

The Director hums in curiosity after quickly scanning over the individual’s application, noting that they are wishing to take over the Hospital Director position

“A very noble role. How interesting. Are you aware that if you are to take over my role within the Norowarejima Emergency hospital, you will also be taking over my previous work and responsibilities?

Nodded her head in affirmation, a small smile upturned the corner of her lips. “I have done my research of the job description and I am well aware and understand that I will be taking full responsibility of your previous work and responsibilities” Tilted her head slightly as she continued to speak. “I know how much will be under my belt but I am confident in being able to handle and organise it well”

 

They clear their throat, collecting a document with a collection of questions they then begin to recite.

Neatly and swiftly placed her documents back in her folder, plopping it down besides her feet - her hands rested back in front of her as she adjusted her posture, prepared for the next wave of questions.

 

“Why should you receive this position above anyone else interested?”

“I was a natural born leader, so this role would be second nature to me. I do also have a lot of experience and study under my belt compared to most, which I can use to my advantage when pursuing this role, but mainly my past experience as a Clinical Manager is what in my opinion makes me stand out from the rest, throughout that role I worked closely alongside the director and the other managers and leads to create policies, protocols and training guides for the hospital. Not only that but I managed my own doctors department within the hospital, giving a little taste of what it would be like in running the entire hospital.” Huffed proudly to herself as she spoke. “I am confident in my skills and ability to thrive in this role and truly allow the hospital to flourish in its upcoming development.”

 

“Now. As the director you will be in charge of not only ensuring your patient’s safety, but managing the safety within the workplace. Let's see.. How would you address a conflict between two Hospital Staff members?”

“I’ve dealt with matters of such in the past so usually I take these conflicts seriously. I’d begin by holding a meeting with both individuals with the respective department leads present as well to light touch on the issue and give them both the opportunity to explain each side of the problem. After that we will all attempt to come up with a fair outcome and resolve the issue there and then. Both staff members will be issued with a warning and if it was to continue afterwards that strikes and possible removal will occur.” She’d clear her throat, once again adjusting her posture. “To ensure patient’s safety, it is important that staff are focused on their patients before anything else so conflict between staff members will not be tolerated.”

 

“The hospital receives a lawsuit from a patient that believes their operation or treatment was not properly executed. Who would you reach out to and how would you handle the lawsuit accordingly?”

“A thorough internal investigation by myself and the leads will begin, we will review the records and treatment notes accordingly and try and uncover what was done wrong. Assuming the hospital has its own lawyer for matters such as these, I’d reach out to them with the information from our investigation and hold a meeting with the involved staff and leads. After the meeting we will ensure that if the patient’s complaint is valid that it is voiced to the hospital as a whole and further training will be given if required. We will follow the legal advice given by our lawyer and pay out the lawsuit accordingly.” Intertwined her fingers together as she spoke this was a matter she hopes to not have to face in her future career.

 

“The Inarigetsu Academy brings in a collection of students studying in the medical field and they wish to witness an operation for an assignment. How will you respond and thus, how would you ensure everyone’s safety and focus during the operation?”

“I will always encourage students wishing to future their academic endeavours and hands on experiences like so are definitely a great learning experience. My answer would be tailored to the specific day and the surgeries that we have ongoing at their arrival. I’d ensure that what they get to witness is a minor surgery only and that it is done with not only the consent of the patient but the surgeon and operating team. To ensure the safety and focus during the operation, the students will be required to wear the appropriate PPC and stand far back from the operating table. These viewings should be limited to only 1-3 students at a time and if anything were to change throughout the operation that they would be asked to leave immediately.”

 

“What unique Contributions can you bring to our hospital in my stead?”

Her eyes lit up as this was the one topic she was most excited to speak about. “I put a lot of thought into what I wish to implement into the hospital but I’ll start out with the basics for now. I’d like to create a brand new training guide for our staff tailored to each department that includes the protocols and requirements for their roles that is also based on a modern approach to medicine and practice. Getting into the specifics though. .  I wish to implement group sessions into the psychiatry department to reach out to the community” She’d nod her head in affirmation at her own idea, proudly flipping to the next page of her document. “Speaking of community engagement, I'd absolutely love to start volunteer medical trials and blood drives. I believe it would put us in a good light with everyone and it’ll help aid our care and treatment for the future. Staff wise, I would create a reward system for everyone for achievements and overall great efforts they put into their job” - “It’ll keep great morale and give them the appreciation that they deserve every now and then!” She’d let out one final hum before concluding. “I have far too many ideas to simply discuss here and if given this amazing opportunity, I’d love the chance to do it in front of my new staff and hear their input!”
