OOC SECTION
[This section is a requirement to properly identify you during your application.]

What is your IGN?:

st4rl0v3r

What is your Discord?:

loverofstars

What would you like us to call you?:

Star is preferred, or Elliot.

How old are you?:
(If you do not want to state your exact age, confirm if you are above the age of 16)

I am 16 years of age.

Do you have a functional Microphone?:

I do.

What is your timezone & How would you describe your activity?:

I am of an EST timezone, and I’m as active as I can be, aka; whenever I have free time, and I am not focusing on something else, I’d be free to log on! Following the week after this application, I’ll also be on break meaning that I’ll be able to be around a lot more often then previously, excluding my own plans.

What is your motivation to apply to the Norowarejima Hospital Faction?:

To learn more, in previous other servers I have been apart of other hospital roleplay scenarios, yet I never thought that I learned all that I wanted to during the time due to lack of leadership, and I fully believe that being apart of being tasked with research will also help in my own ways, as Psychiatry, which is what I am applying for; is also my own workforce that I hope to join. As well as also being in my own process within schooling, and outside of it within my own research.

ROLE-SPECIFIC QUESTIONS & MEDICAL TRIVIA
[This section is dedicated to testing your knowledge on the medical field.]
-
What role would you prefer and why?:
(You can find the list of departments available in the informational section.)

Psychiatrist, own personal workforce that I wish to join in the future, and I believe I will get the most out of being apart of the department and I’ll be able to have more motivation and more activity to be able to assist.

Do you have any prior experience with MedicalRP?:

As stated previously, I was very previously apart of another hospital roleplay; where I wasn’t around for long, yet I do have a high focus in my own MedicalRoleplay as I do find it fun and interesting, and I’d be able to learn more about it.

Are you aware that with MedicalRP comes the possibility of encountering possibly triggering topics?:

Yes, I am aware of this, and have taken this into consideration.

How well can you coordinate with a team during Roleplay?:

During roleplay, I am able to coordinate with people well.

MEDICAL TRIVIA

(PHYSIOLOGY)
If someone is experiencing a reaction to food poisoning, what is the correct way to manage and treat the symptoms?:

Assess how severe the food poisoning is, and help anyway that is possible, administering an IV for fluids if dehydrated, monitor their vitals to make sure that their state doesn’t worsen and refer them for labs if that is needed.

What procedure would you follow if you witness a patient experiencing a seizure?:

Turning them on their side, and making sure there is nothing around them that they could hurt themself on while timing how long the seizure is, as if its over five minutes long, it can cause long-term affects to the body; like hypoxia due to lack of oxygen.

What procedure would you follow if a patient that has entered the hospital has a broken forearm?:

If I am the only one available then I’ll handle it myself, if not; pass it off to somebody who can take better care of the patient.

First, you need to splint the arm (temporarily) before continuing to do anything to make sure it doesn’t get worse, and make sure the patient still has feeling in their hands, and able to feel the pulse via their wrist.. Due to me obviously not being somebody that would be able to have surgery, if it is a bad break; I’d wait for somebody that is able to take care of it properly, if I am able to take care of the breakage myself, I’d X-ray the arm before giving them an IV - if in pain to administer pain meds - and apply a splint or cast, depending on the severity of the case.

What procedure would you follow if a patient enters the hospital struggling to breathe?:

Sit them upright and check if they have any sort of medication on them to be able to assist them in breathing in a case of an asthma attack; checking to see if there is something blocking their airway and checking their pulse and oxygen levels.

(PSYCHIATRY)
A patient is experiencing a panic attack before an important procedure. How would you assist the patient?

I’d help them in anyway I can, and know of; like destressing techniques, assuring them that they are safe and the procedure will be quick and painless; and the symptoms that may follow are only temporary. If it is serious, administer sedatives to assist in de-stressing,

A belligerent patient in the middle of a psychotic episode stumbles into the emergency room. What should you do?

Escort them out of the emergency room into a private room and talk to them there if able to, it’s better to have them away from the public during an episode to be able to help calm them down and keep others safe, including themself.

Which neurotransmitter is most commonly associated with depression

Serotonin and Norepinephrine, which they are both highly affective with mood regulation.

What does the Acronym [PTSD] stand for, and what does it entail

PTSD; Post traumatic stress disorder, is a mental health condition that normally exhibits after witnessing a traumatic event; most seen during wars, which where the affected person will have flashbacks to such events, believing where they are back in the moment; completely reliving the scene, which highly effects daily life and how they function; constantly having the event on their mind.

IN-CHARACTER SECTION
[Everything beyond this point must be answered from an ICLY standpoint]
[This section should be written like a cover letter and professionally formatted and addressed to the Hospital Hiring Committee]

Dear Norowarejima Hospital Hiring Committee,

I am currently writing this letter to express my interest in joining your team as a Psychiatrist. With my previous education, maining in Psychiatry, and branching off into Forensics work, sociology and even art therapy, I believe I’d be a helpful person to add to the team with my extensive term of knowledge. Taking into account my learning while growing up, and having my full interests being apart of helping others, and bettering them as people; not just within the clinical side of things. Having spent time at a previous hospital within my residency, I spent most of my time helping patients through grief and trauma that they experienced first handedly. I hope that my background, experience and my own values will be able to help Norowarejima Hospital.

Thank you for reading through this, and I would be honoured to be able to speak about my achievements and values going forward.

Warm regards,
Sayoko, Nii.
PhD

THE INTERVIEW
[Congratulations! Your character scored an interview, please answer the following all in ICLY fashion]

The Hospital Director takes a seat at their desk, clasping their hands together with a calm gaze as they greet the individual. Their desk is clean and pristine, aside from a half-filled tea cup that continues to steam, giving the room a lovely aroma. “Good afternoon. I will be conducting your interview today. Could I have your full name as well as your age and date of birth, please?”
-
Upon taking a seat, she discretely pushed her glasses up her nose while giving the Director a short bow of her head, her sight wavering around the room to see what decor they had on their walls, what degrees they may have to accompany their walls like trophies. “Good evening. . My name is S-Sayoko Nii. . I am thirty-six, my birthday is October 14th 1988. .”

“Very nice. Now, could you tell me a little more about yourself? Do you have a family, perhaps some motivations behind why you’d like to work here?”
-
Staring down shortly at the desk, staring at the tea cup that was half-filed, she began speaking once again, her voice stuttering through yet filled with confidence with each slip up. “I recently l-lost m-my l-late-husband, as he is currently behind bars. . and was a victim of his own crimes, having also l-lost my daughter three years ago. .” Pausing, the subject of the loss was something hard for her to swallow. “M-my m-motivations. . Simple- I’ve always been a good listener, and was always able to give feedback. . no m-matter the topic, I’ve been told I have a good word for comforting the uncomfortable. .”

“Would you like us to refer to you by your surname or first name? As well as what honorifics you’d like.”
(San, Chan, Kun, Shi, etc.)

Letting a shallow smile grace her face, “I p-prefer being referred to as S-Sayoko. .” Glancing around again for a moment, as to think as she straightened her posture, letting her hands rest neatly on her lap, her head still dipped just a bit, “S-Sayoko-S-San is p-pre-preferred. .”
The director nods in affirmation, now understanding the type of person the individual was and readjusts their posture, taking a sip of that aromatic green tea before continuing to speak.

“It is nice meeting you, then. Now, Let us move on to some more important questions regarding your role.” - “A patient arrives with a severe injury and is panicking uncontrollably. Walk me through how you would handle this situation.”

Nodding, listening along to every word. Story-time, is it? “Regarding m-my role. . I would attempt to calm down the p-patient while a surgeon, doctor, whoever is taking the p-patient, to take care of them better. Attempt to comfort them. . yet out of my own role, I’d attempt with what I could of my own m-medical knowledge, I would help them in any way p-possible that I can, especially if I am for s-some reason the only person around. No p-person will die while in m-my care or in m-my hands. What I'd do in s-specifics would depend on the s-severe injury. Care before comfort, if they are able to happen at the s-same time then they are equals.”

“During an emergency, if any other hospital staff are unavailable however there is a patient in need, can we trust you to stabilize them or treat them to an acceptable degree?”

Staring ahead for a short moment, she let a smile grace her lips as she pressed her hands against her dress very briefly, smoothing the non-existent wrinkles.. “I’d be able to do what in m-my best ability to be able to st-sstabilize them, and if I believe I won’t be able to treat them, I would sstabilize them until somebody is free and able to treat the patient. .”

“Could you run me through on the plethora of equipment you may find within your personal office if you are to be given the role of a resident? Just simply medical equipment.”

“Well… it’d be in mm-my best interest to have s-some ss-smmall-leveled doctor equipment as well as for my own role, first aid, anything of need.. Books, one for my own s-studying, and also books that would be able to possibly help patients with their own mm-mental health, or their own organization.. Testing kits, anything that I would need with a specific patient, or on my own time with helping my own studies.”

I am in the process of getting her a skin that is more accurate to her older character design, I do have a temporary skin for her until then.
